frederick andrew, with one aim; to provide women with financial assistance during times of convalescence. since its inception, the trust has continued to provide grants and is still focused on providing working women and those that have retired from active work with the means to convalesce. the frederick andrew convalescent trust understands that it must change to suit the requirements of today’s women and offers grants of up to £1000 for convalescence and domenstic help and up to £600 for therapy, to those of all ages, from all backgrounds and with almost any medical condition. for details on how to apply for one of our grants please complete the assessment form below. how we can help providing that you are female and have been in paid employment at some time, fact can award a grant to help with the cost of getting better following an illness or injury. grants can be used towards the cost of: convalescence in appropriate accomodation domestic help to aid recovery at home receiving therapy from health and care professions council (hcpc) registered therapists. types of therapy covered include: physiotherapy occupational therapy speech therapy chiropody / podiatry assessment form address which of the following do you require? convalescence home help therapy further information: the trust can only award grants to women. this excludes husbands, partners, children, or other members of your family. if you are intending to convalesce in shared accommodation, eg your family are going with you, please indicate on the form, the amount in respect of yourself only. applications are sent out to three of the trustees separately, in batches so you may not have a response immediately. however, we do respond to every application. privacy statement guidance frequently asked questions what types of grant are available? grants are available for: a convalescent break help in the home therapy by a health and care professions council registered therapist i have previously been awarded a grant can i re-apply? a grant cannot be awarded within 2 years of a previous grant, even if that grant was not taken for any reason within the 6 month period the grant remains available. a further grant will only be considered in exceptional circumstances and priority will be given to those who have not previously received a grant. application for home help what type of help can i apply for? grants are available for help with: cleaning gardening ironing cooking how long will help be available for? if you have undergone a major operation home help can be claimed for 6 weeks following the operation if you have undergone a minor operation home help can be claimed for 2 weeks following the operation if you have a continuing medical condition your application will be dealt with on its individual merits when deciding what period home help can be claimed for what information do i need to supply? you will need to supply details of the following: type of help the hourly rate of that help the hours per week required the number of weeks required if i am successful how is the grant paid? the grant can be paid either: directly to the home help on production of an invoice confirming the details set out above to you on production of a receipted invoice confirming the details set out above grants for domestic help must be paid within the timescale specified at the time the grant is awarded. any grant not paid within this timescale, or any balance of grant remaining, will no longer be available. application for a convalescent break it must be clearly demonstrated that your stay is a convalescent break and not a holiday where can i stay? grants can be awarded for convalescent breaks in the following types of accommodation: convalescent home bed and breakfast accommodation hotel i have found suitable accommodation what details should i supply? you need to provide: the name and address of the accommodation the dates of your intended break a breakdown of the total cost of the accommodation and any travel costs, including details of mode of transport, to and from the accommodation the total amount requested can my family/friends accompany me on my convalescent break? a grant can only be awarded towards the applicant’s cost of the convalescent break. if you are accompanied on the convalescent break a detailed breakdown of the cost must be provided so that only the applicant’s proportion can be calculated. can i stay with family or friends? if you are staying with family or friends a grant can only be awarded towards the cost of travel to and from the location. can i request a grant for a foreign convalescent break? this will only be considered in exceptional circumstances. you must be able to demonstrate a clear benefit in taking a convalescent break abroad rather than in the united kingdom. grants will only be considered for short haul flights (a maximum of 3 hours) if you are intending to stay with family or friends in a foreign country a grant can only be awarded towards the cost of your travel to and from that convalescent break when can i take a convalescent break? if you have undergone minor surgery the convalescent break must be taken within 2 weeks of the date of your operation. if you have undergone major surgery the convalescent break must be taken within 6 weeks of the date of your operation. if you have a continuing medical condition your application will be dealt with on its individual merits in deciding when the convalescent break should be taken, although must be taken within 3 months of the grant being awarded. when should i apply for a grant? in the case of planned surgery you should submit your application as soon as you have been given a date for admission to hospital. a decision will then be given in principle and, if you are successful, payment of the grant made closer to the time. can i apply for a grant to cover the cost of a convalescent break i have already taken? grants are not available for convalescent breaks which have already been taken. if i am successful how is the grant paid? grants can be paid either: directly to the accommodation on production of an invoice to you on production of a receipted invoice grants must be paid and convalescent breaks taken within 6 months of award. any grant not paid within this timescale, or any balance of grant remaining, will no longer be available application for therapy what types of therapy are covered? the therapist must be registered with the health and care professions council (‘hcpc’) details of registered therapies are available on www.hcpc-uk.org/aboutregistration/professions what information do i need to supply on applying for a grant? any therapist registered with the hcpc will have a registration number and expiry date of their registration. these details must be provided name of the therapist type of therapy number of sessions required and estimated cost of therapy without any of these details we will not be able to proceed with your application if i am successful how is the grant paid? a grant will be paid on production of an invoice detailing: the date of each therapy session name and hcpc registration number of the therapist carrying out the treatment do i have to pay for the treatment and claim this from fact? payment can be made: directly to the therapist on production of an invoice detailing the points set out above to you on production of a receipted invoice detailing the points set out above do i claim after each treatment? depending on the number of sessions awarded claims should be made at monthly intervals during the course of the therapy if this will cause financial hardship alternative arrangements can be made grants for domestic help must be paid within the timescale specified at the time the grant is awarded. any grant not paid within this timescale, or any balance of grant remaining, will no longer be available. i did not attend a scheduled appointment, will fact cover the cost? fact will not be responsible for payment of any consultation or treatment sessions where an applicant has failed to attend referee details my carer is related to me can i still give their details as a referee? the referee cannot be a relative of the applicant. please find an alternative referee the referee must complete and sign the referee section of the form and provide a supporting statement please note we may contact your referee to verify the information given within your application form medical practitioner’s statements who can sign this section of the form? ideally the form will be completed and signed by your gp if you are undergoing treatment at hospital the form can be completed and signed by your consultant what should the practice stamp consist of? the practice stamp must detail: the name of the medical practitioner who has signed the form. applications which do not include this information will be returned the practice/hospital from which they work the signature on the medical practitioner’s statement is not dated, what should i do? applications must be submitted within one month of the date on the medical practitioner’s statement. if this section of the form is undated the form will be returned details of why the convalescence/therapy is needed why do i need to complete this section? it is essential, when considering your application, for the trustees to have as much information as possible as to your medical circumstances and how you would benefit from the help requested if the trustees do not have enough background information your form may be returned for further information to be added, this will inevitably delay any decision processing your application how is a decision made? your application will be sent to three trustees who will each decide whether an award should be granted and, if so, the amount to be awarded the responses of the three trustees are then considered alongside each other and an award granted or refused on the basis of the comments made the decision of the trustees is final, there is no appeal process how long should the process take? applications are submitted to the trustees in batches of 5 or 6 at a time fact aims to let you have a decision within 4 weeks of receiving your application testimonials
